Finally! Some great form designs

I know they are out there. I've been looking for example pages with great form designs on them, and finally I found Superb Examples of Form Design from Inspect Element. HTML forms don't have to be ugly. Do you have suggestions for sites with great form designs? Post the URLs in the comments!
