About This Free Web Template:
This template uses tables to create a Web form with labels on the left and lined up down the left side of the form. This is valid XHTML because forms are a type of tabular data and so can be displayed using tables.

Details of Basic Web Form with Left Aligned Labels Template:
- Form fields are positioned using an HTML table
- Checkbox and radio button groups in unordered lists
- CSS defines the layout and design
- Labels are blue and slightly larger to make them easier to see
To Use the Free Web Template:
- Copy the CSS into a document named styles.css
- Copy the HTML into a file in the same directory
- Don't forget to add a title to the page
- Publish all documents to the same directory on your Web server (HTML page, image, and CSS file)
- If you want to add design changes - create and link to another CSS file or add your styles to styles.css
